CARE
Care for individuals nearing end of life meaning with 6 months or less to live

Mission
Hospice care is a specialized, comfort‑focused model of medical care for people who are nearing the end of life—typically when their life expectancy is six months or less, and curative treatment is no longer beneficial. Its purpose is to support both the patient and their family physically, emotionally, and spiritually, allowing the person to live their final months with dignity and as little discomfort as possible.

Service
Hospice is medical care for people expected to live six months or less, provided primarily where the person lives—at home, in assisted living, or in a nursing facility. The focus shifts from curing illness to managing symptoms, relieving pain, and supporting quality of life.
Hospice teams address the whole person, not just the disease, coordinating care around the patient’s goals, values, and comfort.
Services include but aren't limited to:
-
-
Medication supervision for pain and symptom control
-
Medical equipment (hospital bed, wheelchair, oxygen)
-
Medical supplies (bandages, catheters, etc.)
-
Therapies (PT/OT/speech) when appropriate
-
Short‑term respite care for family caregivers
-
Grief and bereavement support for over a year after death
-
These services are available 24/7 for support or urgent needs.
