who qualifies for funding?

There are very specific requirements in order to receive funding assistance. We are blessed to have a non-profit organization as our trusted sponsor, that allows these donations to be tax exempt. The ICCF Alliance is hosting our Fundraising Campaign called Bridge the Gap. All funding goes through an application process but first you must know the criteria to qualify. Please read them carefully before applying.

Applicant Terms & Conditions

Disclaimer: Term & Conditions may change as other Home Care companies come aboard the Bridge the Gap Campaign, but until then, these are the set terms.

To qualify for assistance, applicants must demonstrate financial hardship, reside in South Carolina, and have a verified need for in‑home care. Eligibility is based on household income at or below 200% of the Federal Poverty Level, lack of available caregiver support, and documentation from a physician or care assessment confirming the need for personal care or supervision. Assistance is limited to a maximum annual award determined by Burton Home Care Services and may only be used for approved non‑medical in‑home care services. All applications require proof of income, residency, and medical need, and are reviewed by an internal committee.

1. Financial Need Requirements

These are the most important and must be objective.

You can require all of the following:

  • Household income below a set threshold Most nonprofits use 200% of the Federal Poverty Level (FPL). For example, in 2026, 200% FPL for a single person is about $30,120.

  • Proof of financial hardship, such as:

    • Recent pay stubs

    • Social Security award letter

    • Unemployment documentation

    • Bank statements

    • Medicaid denial or pending status

  • No available family caregiver able to provide the needed care

This ensures funds go to people who genuinely cannot pay.

2. Care Need Requirements

You must define what “needs in‑home care” means.

Require at least one of the following:

  • A physician’s note stating the person needs assistance with daily living

  • A home care assessment showing need for personal care, supervision, or mobility support

  • A diagnosis such as:

    • Alzheimer’s or dementia

    • Stroke recovery

    • Mobility impairment

    • Chronic illness requiring assistance

    • Post‑hospital recovery

This prevents people from applying who simply want convenience care.

Choose one of these structures:

Option A — Seniors Only

  • Must be age 60+

Option B — Seniors + Disabled Adults

  • Age 60+ OR

  • Adults 21+ with a qualifying disability or chronic condition

This widens your impact while staying mission‑aligned.

5. Limitations on How Much Help They Can Receive

To protect your fund from being drained:

  • Maximum award per person per year: Example: $500–$2,000 depending on your budget

  • Maximum number of hours covered: Example: 10–40 hours of care

  • Funds may only be used for:

  • Funds may NOT be used for:

    • Rent or utilities

    • Groceries

    • Cash payouts

    • Skilled nursing or medical procedures

    • Care provided by family members

6. Documentation Required

To keep everything clean and auditable:

  • Completed application

  • Proof of income

  • Proof of identity

  • Proof of residency

  • Medical or care‑need documentation

  • Signed release allowing Burton Home Care to verify information

7. Review & Approval Process

To avoid favoritism or liability:

  • Applications reviewed by a 3‑person committee

  • Decisions made within 10–14 days

  • Applicants notified in writing

  • Appeals allowed once within 30 days

In‑Home Care Assistance Fund Eligibility Policy

1. Purpose of the Fund

The Bridge the Gap Assistance Fund provides limited financial support to individuals who cannot afford essential non‑medical in‑home care services. The purpose of this policy is to establish clear, fair, and consistent eligibility criteria to ensure funds are distributed to individuals with the greatest financial and care‑related need.

2. Eligible Services

Assistance may only be used for non‑medical in‑home care services provided directly by Burton Home Care Services, including:

Funds are never provided directly to applicants. All services are delivered by Burton Home Care Services staff.

3. Applicant Eligibility Requirements

3.1 Residency

Applicants must:

Applicants must meet one of the following:

  • Age 60 or older, or

  • Age 21+ with a qualifying disability, chronic illness, or functional limitation requiring in‑home support

Applicants must demonstrate financial need through all of the following:

  • Household income at or below 200% of the Federal Poverty Level (FPL)

  • Inability to pay for in‑home care services

  • Lack of available family or informal caregivers able to provide adequate support

Acceptable proof of income includes:

  • Recent pay stubs

  • Social Security or disability award letters

  • Unemployment documentation

  • Bank statements

  • Tax returns

  • Medicaid denial or pending status

3.4 Care Need Verification

Applicants must demonstrate a legitimate need for in‑home care through one or more of the following:

  • Physician’s note or medical documentation

  • Home care assessment conducted by Burton Home Care Services

  • Diagnosis indicating functional limitations (e.g., dementia, mobility impairment, chronic illness, post‑hospital recovery)

4. Limitations of Assistance

4.1 Maximum Award

To ensure fair distribution of funds:

  • Maximum assistance per individual is $500–$2,000 per calendar year, depending on available funding

  • Maximum number of hours covered is 10–40 hours, determined by assessment

4.2 Non‑Covered Uses

Assistance cannot be used for:

  • Rent, utilities, or household bills

  • Groceries or personal shopping

  • Skilled nursing or medical procedures

  • Cash payments to applicants or family members

  • Services provided by any provider other than Burton Home Care Services or authorized provider associated with Bridge the Gap

5. Application Requirements

Applicants must submit:

  • Completed application form

  • Proof of income

  • Proof of identity

  • Proof of residency

  • Medical or care‑need documentation

  • Signed release authorizing Burton Home Care Services to verify information

Incomplete applications will not be reviewed.

6. Review and Approval Process

6.1 Review Committee

Applications are reviewed by a three‑person internal committee appointed by Burton Home Care Services leadership.

6.2 Decision Timeline

Applicants will receive a written decision within 10–14 business days of submitting a complete application.

6.3 Approval Criteria

Decisions are based on:

  • Financial need

  • Level of care required

  • Availability of funds

  • Risk factors (e.g., living alone, fall risk, cognitive impairment)

6.4 Appeals

Applicants may submit one appeal within 30 days if new information becomes available.

7. Duration of Assistance

Approved assistance is provided for a limited period:

  • Typically 1–4 weeks

  • Extensions may be granted based on reassessment and available funding

All cases are subject to periodic review.

8. Confidentiality

All applicant information is confidential and used solely for eligibility determination and reporting required by law or donors.

9. Program Integrity

Burton Home Care Services reserves the right to:

  • Verify all information submitted

  • Deny or discontinue assistance if false information is provided

  • Modify this policy as needed based on funding availability or organizational requirements
